Why does this matter for answering questions?
Because it settles most 'what does the nurse do first' items. When several options are all reasonable actions, the one earliest in the process is usually correct — you assess before you intervene unless the situation is immediately dangerous.
When do I skip assessment and act?
When waiting would cause harm. Airway, breathing and circulation problems are acted on rather than assessed further. Outside those, gathering information before intervening is the safer read of a question.
How is this different from prioritisation?
The nursing process orders your steps for one patient; prioritisation orders patients against each other. Questions often combine them, so know which one is being asked before choosing.
